Minimum Qualifications of Education and Experience

Bachelor’s degree from an accredited college or university in Nursing.

Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver’s license and be free of any major traffic violations for the last three (3) years.

Required Special Qualifications:

Licensure as a Registered Nurse in the State of Minnesota is required upon employment.
Certification as a Public Health Nurse in the State of Minnesota is required within six (6) months of employment.

Nature of Work

This position provides assessment, consultation and referral services to individuals and families in the community. Works primarily within the Adult & Family Services Division.  Work assignments to include Case Management for Elderly Waiver, Alternative Care and Essential Community Supports programs and Elderly Waiver Care Coordination for Managed Care Organization contracts for individuals over 65 years of age. Supports a diverse, respectful, and inclusive workplace.  Must be available to support the department during emergency situations.

Examples of Work

With an emphasis on prevention and assisting individuals to remain independent within the community, conducts comprehensive, community-based health assessments
Provides case management and care coordination services to older adults per DHS and health plan guidelines
Works as a resource to individuals and their families in order to help them understand the implications of medical diagnosis and support them with carrying out treatment plans
Arranges services needed by clients and coordinates the delivery of these services
Conducts assessments of individuals to determine eligibility for public funded programs
Makes referrals to service providers as appropriate
Provides appropriate health promotion and education information to clients
May assist epidemiology disease surveillance, investigation activities, and participate in public health disaster preparedness
Performs related duties as assigned

Knowledge, Skills and Abilities Required

Knowledge of the application of public health nursing theory and practices
Skill in working with older adults with chronic conditions
Ability to assume responsibility for the appropriate judgement in community care settings
Ability to establish and maintain cooperative and meaningful relationships with clients, client families, agency staff members and representatives of other agencies, and the general public
Ability to apply previously acquired knowledge and skills to new situations
Ability to accept supervision and guidance
Ability to communicate effectively, both verbally and in writing
Ability to make independent decisions
Ability to work effectively with community agencies
