We are actively seeking compassionate, motivated, and service-oriented individuals to join our team as Remote Health Coaches / Care Coordinators. In this role, you will engage directly with clients to provide personalized wellness guidance, care coordination, and encouragement. By supporting lifestyle changes, promoting preventive care, and connecting individuals to the right resources, you’ll play an essential part in improving health outcomes and empowering lives.

What You’ll Do

Conduct virtual outreach and coaching sessions with clients to encourage healthy behaviors and support their personal wellness goals
Guide individuals through chronic condition management, preventative care, and lifestyle improvements in areas such as nutrition, physical activity, mental wellness, and stress reduction
Help clients access appropriate healthcare providers, services, and community support based on their needs
Maintain accurate records, track progress, and update wellness plans through digital health platforms
Educate and motivate individuals with kindness, clarity, and empathy—tailoring support to diverse needs and backgrounds
Collaborate with healthcare professionals and internal teams to ensure a coordinated, holistic approach to care
Identify and help address social or environmental factors affecting clients’ well-being

Who We’re Looking For

Preferred Qualifications (Entry-Level & Experienced Welcome):

Degree or coursework in Health Sciences, Public Health, Psychology, Social Work, Nursing, or a related field
Prior experience in health coaching, customer service, care coordination, or community outreach is a plus—but not required
Strong verbal and written communication skills with a passion for helping others
Comfortable using technology for virtual meetings, documentation, and scheduling
Able to work independently in a remote setting while managing time effectively
Bilingual candidates are strongly encouraged to apply
