Position Summary:
The Remote Community Health Worker (CHW) will serve as a trusted liaison between community members and healthcare resources. The CHW will provide culturally competent health education, assist individuals in accessing services, and support community-based outreach initiatives to improve health outcomes.
Key Responsibilities:
Engage with community members to assess needs and connect them to health and social services
Deliver health education and wellness guidance to diverse populations
Conduct outreach activities including workshops, home visits, and community events
Advocate for clients and address barriers to care
Maintain accurate records and support program evaluation and reporting
Collaborate with multidisciplinary teams to enhance program impact
Required Qualifications:
Experience in community outreach, public health, social services, or healthcare support
Strong communication and interpersonal skills
Ability to work effectively with diverse populations
Highly organized with attention to detail
Bilingual skills preferred
Preferred Qualifications:
Knowledge of local health resources and public health systems
Experience with program evaluation or health data tracking
Compensation & Benefits:
Annual salary: $55,000 – $70,000 (commensurate with experience)
