Job Description and Duties
This position supports the California Department of Public Health’s (CDPH) mission and strategic plan by supporting chronic disease evidence-based intervention programs that promote healthy behaviors, healthy communities, and improve the prevention, diagnosis, and management of chronic disease. This position is eligible for telework in accordance with CDPH telework guidelines.

 

The Health Program Specialist (HPS) I works under the general direction of the Health Program Manager (HPM) II, Program and Policy Section Chief. The HPS I serves as a highly skilled, technical program consultant for Chronic Disease Control Branch (CDCB) programs. The HPS I works collaboratively with an internal multi-disciplinary team, and develops and oversees contractual relationships and partnerships with health jurisdictions, health systems, and community-based organizations to implement, monitor, evaluate, and report on programmatic strategy implementation and deliverable completion. The HPS I coordinates communications, meetings, trainings and technical assistance (TTA), and project meetings with internal program staff and external contractors and partners.

 

The HPS I serves as a subject matter expert (SME) in team-based care, supporting the engagement of non-physician team members, including pharmacists and community health workers, to promote prevention and management of chronic diseases amongst California’s diverse people and communities, improving health equity and addressing social needs and social determinants of health. In collaboration with program team staff, the HPS I plans, develops, authors, and submits progress reports, semiannual reports, and continuing funding applications to the CDC and other funding organizations. The incumbent is a CDCB website team member, researches and responds to drills, legislative inquiries, and public inquires, provides bill analyses, and authors legislative and/or budget proposals as needed. This position requires travel (in state and out of state) approximately 10% of the time to attend conferences and trainings.

The attached duty statement indicates whether this position is eligible for telework. All employees who telework are required to be California residents in accordance with Government Code 14200, and may be required to report to a CDPH office, when needed. Candidates who reside outside of the state of California may be interviewed; however, the selected candidate must have a primary residency in the state of California prior to appointment (and continue to maintain California residency) as a condition of employment. Failure to meet this requirement may result in the job offer being rescinded.
