Job Description
Position Summary:
Under the general supervision of the Health Services Coordinator, this individual functions across all components of early intervention services including HIV and HCV testing, referral services, PrEP (pre-exposure prophylaxis)/PEP (post-exposure prophylaxis) navigation, community engagement, syringe services (SWAP), data collection, and linkage to care. This position covers Region 5 (multi-county), requiring in-state travel and evening/weekend hours based on client needs. The individual collaborates closely with the HIV Prevention Team to avoid duplication of services and supports harm reduction and health equity principles. This position demands cultural sensitivity, independent judgment, and handling of confidential, sensitive information. This position is 100% grant funded.
The employee receives periodic review and appraisal of performance plus functional and administrative supervision. The employee will be responsible for confidential and personal information and must adhere to the confidentiality and privacy policies of the department.
Essential Duties & Responsibilities:
PrEP Navigation & PAPI Coordination
- Act as PAPI (Prevention Assistance Program Intervention) Coordinator; maintain and grow provider network.
- Enroll clients in PAPI and assist with PrEP/PEP access.
- Maintain accurate client records and billing in the PAPI system.
- Follow up on insurance, medication continuation, and biomedical prevention.
HIV and HCV Testing and Referral
- Provide testing during clinical hours or per client availability.
- Provide prevention or treatment referrals, including to the Disease Intervention Specialist.
- Ensure no service duplication with other HIV Prevention Team members.
Case Coordination and Community Collaboration
- Establish referral points from high-risk agencies (e.g., Emergency Departments [EDs], shelters, Federally Qualified Health Centers [FQHCs]).
- Coordinate client services with community partners.
- Develop service plans and refer complex cases to medical case managers.
Data Management and Grant Support
- Track and report grant deliverables and program progress.
- Participate in grant writing, reports, and ODH calls/work groups.
- Maintain effective communication with funders.
SWAP Program Coordination/Support
- Provide non-judgmental harm reduction services.
- Maintain supply inventory and order replacements.
- Co-Facilitate SWAP committee with Harm Reduction Manager and represent SWAP at meetings/events.
- Enter and verify data; prepare reports for stakeholders and the CCPH annual report.
- Be prepared to encounter individuals who may be under the influence of substances and utilize de-escalation techniques to ensure a safe and supportive environment for all clients and staff.
Community Engagement & Public Health Promotion
- Act as CCPH spokesperson at public events and health fairs, and promote CCPH services and HIV prevention/harm reduction initiatives through outreach.
- Participate in marketing and awareness campaigns.
- Advocate for and serve as a resource for marginalized populations (LGBTQ+, People Who Inject Drugs [PWID], Black, Indigenous, and People of Color [BIPOC]).
- Contribute to assessments, visioning, and health improvement initiatives.
Additional Duties
- Participate in internal committees and quality improvement initiatives.
- Provide emergency response support as needed.

Minimum Qualifications and Credentials:
- Bachelor’s degree in Psychology, Sociology, Social Work, Human Services, Public Health, or related field.
- Valid driver’s license with ability to travel up to 25% of the time.
- Strong written/oral communication and motivational interviewing skills.
- Ability to follow laws/regulations and work independently within a team.
- Flexible availability including evenings/weekends.
- Commitment to CCPH mission, vision, values and harm reduction principles.
The following must be acquired and maintained. These credentials are not required upon initial hire but must be acquired after hiring within the probationary period. Training will be provided for the following: Harm Reduction Training, Bloodborne pathogen Training
Work Environment:
- 75% in climate-controlled office, 25% in field across eight counties.
- Work with high-risk populations, including those with addiction and mental illness.
- Program clinic operations and client interactions may entail regular exposure to psychologically or emotionally challenging information, necessitating resilience and a supportive team atmosphere to maintain well-being of self and staff and effectively address the needs of individuals served.

- Evening and weekend hours required
- Must lift up to 25 lbs, climb ladders, work in variable conditions.

RATING
Your name will be placed on a list of eligible candidates upon receiving a passing score. Once the list has been established, your name will remain on the eligibility list for a period of one (1) year.”
Repeating an Examination
Per OAC Rule 123: 1-9-01, an applicant who has competed in a civil service examination cannot repeat that examination or take an examination for the same classification within FOUR MONTHS from the date of original examination, unless an alternative form of examination is given, or unless other standards are specified in the examination announcement.
