Overview
We are seeking a dedicated and empathetic Community Health Worker (CHW) to serve as a vital link between our organization and underserved communities. In this role, you will facilitate access to healthcare and social services, promote healthy behaviors, and empower individuals through culturally responsive education and support.
As a CHW, you’ll work in both the field and with community partners to build trust, address health needs, and improve overall well-being.
Key Responsibilities:
Conduct outreach through home visits, community events, and health fairs to engage and educate residents
Deliver culturally appropriate health education on disease prevention, chronic disease management, nutrition, and public health initiatives
Assist individuals with accessing healthcare and social services, including scheduling appointments, navigating insurances, and completing applications
Advocate for the health needs of individuals and communities, liaising with healthcare providers and social service agencies
Collaborate with internal and external partners—health professionals, nonprofits, and local organizations—to coordinate care and outreach efforts
Collect and maintain accurate records of client interactions, outcomes, and referral data for program evaluation purposes
Provide follow-up and informal counseling, and support behavior change using empathy and active listening
Qualifications & Skills:
High school diploma or equivalent required; Associate’s or Bachelor’s degree in Public Health, Social Work, or related field preferred
Completion of a Community Health Worker training program or relevant certification (if required by state or employer)
Familiarity with the community served, including cultural norms, language, and local health needs
Strong verbal and written communication, interpersonal, and active listening skills
Proficiency in basic computer and recordkeeping tools; experience with data entry or EHRs a plus
Ability to work independently and as part of a multidisciplinary team
