Overview

This full-time contractual job is about helping people with special needs live a full, good life. You will be their main helper as they get services.

You will:

  • Arrange for and coordinate comprehensive assessments.
  • Conduct interpretive interviews with individuals and their families/caregivers regarding findings and recommendations of evaluations.
  • Coordinate the development of the plan for services (PCP or SSPT) and provide on-going monitoring or service plan on a schedule dictated by service category.
  • Locate and link individuals to community resources
  • Maintain confidential records and reports required in a format approved by DDA.
  • Facilitate and complete plan revisions or self-directed budget modifications.

Must have:

  • Bachelor’s degree in a human services field ; OR
  • Associates degree with two (2) years experience in a human services field; OR
  • Seven (7) years experience in a human services field.
