Responsibilities

The Senior Program Coordinator will oversee the coordination of program development, and improvement activities for the National and Innovative Cardiovascular grants In the Heart Disease and Stroke Prevention Program (HDSPP), funded by the Center for Disease Control and Prevention (CDC).  The Heart Disease and Stroke Prevention Program engages health system and community-based partners in planning, implementing and evaluating evidence-based strategies to reduce disparities for patients at risk of cardiovascular disease. The program uses quality improvement strategies to identify gaps in care and implement improvements with partners through a learning collaborative. The incumbent will lead calls and presentations with stakeholders; report on progress and accomplishments to CDC and partners through presentations and progress reports; and work closely with project evaluation and data staff to ensure accurate submission and interpretation of clinical quality improvement data that meets program goals and funding requirements. The incumbent will also oversee contracts with partners.

 

Minimum Qualifications

A Bachelor’s degree in a related field and four years of program coordination experience in a public health, health, or human services-related program or community-based organization; OR an Associate’s degree in a related field and six years of such experience; OR eight years of such experience. The experience must have been contributing toward the development, implementation and/or evaluation of a program, project or other related activities.  A Master’s degree in a related field may substitute for one year of experience. At least two years of experience must have included supervision of staff and/or program management.

 

Preferred Qualifications

Graduate degree in public health, health administration or related field; at least five years of experience in a public health, human service or community-based organization; at least two years of experience in program management; at least two years of experience with cardiovascular evidence based strategies; experience conducting, developing, and monitoring quality improvement initiatives; quality improvement training; experience addressing health equity and health related social needs; experience working with health system partners and community-based organizations; experience with grant writing/grant deliverables and budgets, experience with public speaking/presentations to stakeholders and external partners, experience with supervising staff; experience with contract management; experience with applications including Word, Excel, PowerPoint, teams, zoom/Webex.

 

Conditions of Employment

Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.

Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.

This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting will be available.
