Description

Los Angeles Community Health Project (CHPLA) is a program of Heluna Health. CHPLA’s mission is to improve the health and wellbeing of people affected by substance use in Los Angeles, to increase access to health care, empower people to protect themselves, and educate each other to reduce harm in our communities. CHPLA combines direct services, advocacy, outreach, education, and research collaborations to fulfill this mission. Los Angeles Community Health Project was founded in 1992.

Under the direction of the Director of Client Services, the Clinical Supervisor is responsible for providing clinical supervision to CHPLA’s ICMS LEAD Program (Intensive Case Management Program, formally: Law Enforcement Assisted Diversion/ updated to: Let Everyone Advance with Dignity) that supports both staff and participants. LEAD (Law Enforcement Assisted Diversion) is a collaborative project involving local police, prosecutors, public defenders, corrections officers and social service providers designed to divert individuals arrested or suspected of low-level drug offenses and/or prostitution into social services rather than the criminal justice system. These individuals will be engaged in supportive services with the goal of reducing legal recidivism and improving quality of life.

CHPLA provides harm reduction services across greater Los Angeles through a range of delivery models and person-centered service modalities. The population we serve experience many critical and acute situations that require immediate and clinically appropriate responses. This person will be expected to support case management and harm reduction care services provided by CHPLA’s, as well as ensure clinically sound interventions and quality of care, contractual compliance of care, provide ongoing and regular clinical support to staff, and create and implement sound and effective resolutions to crisis situations.

This is a full-time benefitted position.

The pay for this position is $90,000.00 annually.

ESSENTIAL FUNCTIONS

Program Management

  • Generate monthly reports, collect program data, and monitor contractual compliance benchmarks, performance outcomes, demographics, and integration efforts.
  • Maintain a client roster and assign client cases to program staff who will provide direct services as needed.
  • Support LEAD Program team under health and wellness programs in assessing and consulting about client/participant needs with a true harm reduction, nonpunitive, and clinical approach.
  • Support with clinically appropriate linkages to outside service providers and community partners.
  • Engage in field-based intervention to support case managers with high acuity clients and crises.
  • Track and monitor client medication lists and support medical and psychotropic medication needs.
  • Attend treatment planning and case consultation meetings with partnered providers for the purpose of care coordination.
  • Respond to crisis situations to assess, deescalate, and lead the team in clinical intervention.
  • Submit incident reports to DHS within 24-48 hours.
  • Oversee and support charting, electronic records, and quarterly Care Plans of case managers as required by DHS (Department of Health Services).
  • Assess the needs of program staff to ensure the delivery of relevant and effective case management, harm reduction interventions, and resource navigation.
  • Monitor the progress of program participants; monitor and maintain accurate documentation of supportive services in accordance with Federal, State, County, and agency guidelines.
  • Provide quality supervision regarding case management support, housing navigation, community reintegration, triage activities, and client documentation including eligibility.
  • Accompany and monitor field-based services, as needed, of case management and other program staff.
  • Support LEAD Program team under health and wellness programs in assessing and consulting about client/participant needs with a true harm reduction, nonpunitive, and clinical approach.
  • Support with clinically appropriate linkages to outside service providers and community partners.
  • Oversee drop-in center services and ensure safe, nonjudgmental, welcoming, impactful, and beneficial space for participants.
  • Attend all required trainings and meetings.
  • Attend, advocate, and lead at ongoing court hearings in various districts for participants with their assigned case manager.
  • Participate in scheduled rotation of coverage for 24-hour/7 days on-call crisis intervention hotline.
  • Document interactions and events from 24-hour/7 days on-call crisis intervention hotline. Answer incoming crisis calls, coordinate care and/or resources for clients.
  • Be available some evening and weekends to provide guidance during potential emergency situations encountered by ICMS case management staff.

Community Engagement

  • Work directly with local police departments to build relationships and educate on program with goal that police will divert to services in lieu of arrest.
  • Represent the Agency in a professional manner at meetings and community events.
  • Represent CHPLA at meetings with ODR, law enforcement, and other partners.
  • Must know, understand, and be able to articulate the mission, vision, and core values of CHPLA and harm reduction as a practice.

Team Leadership

  • Troubleshoot with CHPLA leadership team to develop strategies to remove barriers to achieving program/monthly goals and outcomes.
  • Facilitate and participate in all-staff meetings at CHPLA. Openly receive and provide input towards program development and quality management.
  • Provide clinical oversight of LEAD Program case management services provided by CHPLA, staff interactions with ICMS participants, programmatic operations, and participant services documentation, etc.
  • Provide in-person assessment, triage, crisis intervention and problem-solving services for LEAD participants experiencing complex health, mental health and/or substance use challenges.
  • Accompany each case manager during in person meetings with LEAD participants, at least one (1) time every quarter, to assess case manager skills with participants, model interventions for case managers, and assess case manager needs that are to be addressed in ongoing clinical supervision sessions.
  • Conduct regular chart review and case conferences with the County regarding ICMS-HR participants.
  • Conduct regular clinical supervision sessions and case conferencing with ICMS staff.
  • Be available on an on-call basis to provide guidance during any emergency situations encountered by ICMS-HR case management staff.
  • Provide weekly individual and group supervision for up to ten staff and interns as assigned by the Director of Client Services.
  • Provide clinical and administrative supervision of clinical staff (includes ICMS clinical case managers, interns etc.), to ensure delivery of quality services and care and support staff’s clinical knowledge and growth.
  • Plan and participate in special community events held to spread awareness of programs or promote participant health, wellness and empowerment.
  • In collaboration with Director of Client services, recruit, interview, and onboard new employees.
  • Evaluate staff for performance related issues. Address issues in a productive, supportive, and timely manner.
  • Provide support in maintaining a safe, positive, and empowering work environment.
  • Collaborate with Director of Client Services to develop policies and procedures to support teams across the agency and improve delivery of services.
  • Attend community meetings to promote and offer education about ODR (Office of Diversion and Reentry) LEAD and the care options offered through the program.
  • Provide clinical and trauma-informed trainings for CHPLA staff.

NON-ESSENTIAL FUNCTIONS

  • Other duties as assigned by the Executive Director and Director of Client Services

JOB QUALIFICATIONS

Education/Experience

  • At least three (3) years of experience working with people experiencing homelessness (PEH), people who use drugs (PWUD), and/or who have chronic health issues.
  • Master’s degree in social work or similar mental health related major
  • At least two (2) years of experience providing direct social services or Intensive Case Management Services – Harm Reduction (ICMS-HR)
  • Experience supervising a team of case managers (or staff in similar roles, such as outreach, peer support services, or community health workers, etc.).
  • Expertise in the following areas: outreach and engagement strategies, intensive case management services, motivational interviewing, harm reduction, housing first, crisis intervention, suicide assessment and prevention, substance use and mental health.
  • Lived experience and/or diverse background is supported and preferred
  • Experience with working with clients/participants that use drugs, engage in sex work, and/or are unhoused
  • A minimum of 3 years of experience working with crisis assessment, stabilization, and linkage services
  • At least three (3) years of experience in administrative and clinical supervision of staff
  • Progressive clinical harm reduction approach to meet clients and staff needs
  • Deep and thorough understanding of harm reduction, low-threshold services, trauma informed care and patterns, risks and behaviors of drug use.
  • Deep understanding of client assessment, care planning, care coordination, evaluation and advocacy.
  • Understanding of oppression, racism, and disenfranchisement in our society due to power and status hierarchies
  • Understanding of criminal justice, mental health, medical, and housing systems
  • Able to travel and work in the field as needed
  • Bilingual/Spanish-speaking is preferred.

Certificates/Licenses/Clearances

  • A valid California driver’s license, proof of vehicle insurance, and reliable transportation or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions. Must have the ability to drive program vehicles when needed.
  • Must be at least 21 years of age and will be required to enroll in the DMV Pull Notice program
  • Background check clearance required.
  • Active/Valid BBS license as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT).

WORK ENVIRONMENT

General Office Setting. Position requires some traveling, scheduling, attending meetings and client outreach and community engagement.
