Description
The Diversion and Recovery Team (DART) in Hennepin County’s Human Services and Public Health (HSPH) Behavioral Health division is seeking a Social Worker who is passionate about harm reduction, integrated care, and health equity. This position will provide voluntary, intensive case management to Hennepin County residents, who are 18+, living with a substance use disorder (SUD) and experiencing housing instability and has an interest in improving quality of life. You will work on an interdisciplinary team that includes additional case managers, peer recovery specialists, a nurse, and a vocational services worker.
This position will partner with a variety of agencies and organizations to advocate for client’s autonomy and their desired recovery path as client’s move through transitions of care. This often involves an understanding of client’s choice and capacity and routine communication with partner agencies to negotiate a realistic plan of action. You will operate from a harm-reduction position with an ability to comfortably serve individuals in a variety of recovery avenues and support their autonomy and dignity by connecting the individual with community resources that match their goals. You will use a non-judgmental approach to engage with diverse clients, who may be experiencing a crisis related to mental health (MH) and/or a SUD, and have the ability to make quick decisions with the client under limited supervision.

Location and hours:
This position is hybrid, working primarily face-to-face in the community, at 1800 Chicago Avenue South, Minneapolis, MN 55405, and remote as job duties require. Work hours are Monday through Friday from 8 a.m. to 4:30 p.m. Occasional evenings may be required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
· Provide voluntary case management services rooted in a person-first, trauma-informed, harm-reduction approach.
· Develop rapport and form long-term relationships with diverse clients with SUD, assess their evolving needs, engage in creative problem-solving, and deploy de-escalation techniques.
· Advocate for client’s desired recovery path while maintaining positive working relationships with partner agencies, including medical facilities, and other MH, SUD, and/or housing programs.
· Work with a variety of partner agencies, including but not limited to; the Behavioral Health and Wellness Clinic, American Indian Community Development Corporation (AICDC), crisis residences, hospitals, withdrawal management, board and lodges, and substance use treatment agencies.
· Maintain case records and correspondence with an interdisciplinary team, including completing assessments (e.g., needs assessments, functional assessments, and harm reduction assessments).
· Maintain and contribute to policies, procedures, and resources for the staff team.
· Provide support and consultation to other teammates, including but not limited to case consultation, signing the documentation, facilitating workgroups or meetings, or developing tools and resources.
Need to have:
· One of the following:
o Bachelor’s degree or higher in social work, psychology, sociology, human services, or a related field.
o Licensure as a social worker by the Minnesota Board of Social Work.
· A valid driver’s license, daily access to reliable transportation to travel throughout Hennepin County, and the ability to obtain a Hennepin County Driver’s permit.
Nice to have:
· Dually licensed as a Licensed Independent Clinical Social Worker (LICSW) and Licensed Alcohol and Drug Counselor (LADC), or any of the following:
o Current Licensed Graduate Social Worker (LSGW), Licensed Associate Marriage and Family Therapist (LAMFT), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC) and working towards becoming clinically licensed as a Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Clinical Counselor (LPCC) or Licensed Alcohol and Drug Counselor (LADC).
· Experience:
o Serving individuals living with SUD.
o Employing harm-reduction strategies with individuals.
o Serving individuals living with a MH condition.
o Effectively interacting with people from diverse backgrounds including efforts towards disparity reduction.
· Knowledge of:
o County and community resources for MH, SUD, social services, and basic needs.
o Harm reduction and trauma-informed care principles.
o Person-centered thinking.
o Social determinants of health.
o De-escalation strategies.
· Ability to:
o Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.
o Effectively interact with people from diverse backgrounds.
o Work well in a group setting, be accountable to the team.
