Job Description:
Are you passionate about public health, patient empowerment, and equitable access to care? We’re looking for a compassionate and skilled Remote Health Coach / Care Coordinator Specialist to join a collaborative care team focused on improving population health outcomes through personalized support and care coordination.
This fully remote role combines the principles of health coaching, chronic disease management, and social care navigation to help individuals—especially those from underserved communities—achieve sustainable health goals. You will play a key part in promoting wellness, reducing health disparities, and connecting people to the care and resources they need.
Core Responsibilities:
Deliver individualized virtual or telephonic health coaching sessions using evidence-based behavior change strategies, including motivational interviewing
Co-create care plans based on clinical guidance, social context, and the client’s own goals and preferences
Provide education and support for chronic condition management (e.g., diabetes, hypertension, obesity)
Coordinate with primary care providers, behavioral health clinicians, and community-based organizations to ensure integrated and person-centered care
Identify and address barriers to care, including social determinants of health (e.g., housing, food access, transportation)
Track and document client progress, interventions, and outcomes in compliance with HIPAA and data privacy standards
Contribute to quality improvement efforts and population health data reporting
Promote health literacy, prevention, and self-management with a focus on health equity and cultural humility
Required Qualifications:
Bachelor’s degree in Public Health, Health Education, Social Work, Nursing, Nutrition, or a closely related field
Minimum of 2 years of experience in health coaching, care coordination, case management, or community-based health roles
Working knowledge of chronic disease management, motivational interviewing, and health behavior change frameworks
Proficient in using telehealth platforms, electronic health records (EHRs), and digital communication tools
Excellent communication, time management, and interpersonal skills
Strong understanding of public health principles, health disparities, and social determinants of health
Preferred Qualifications:
Master’s degree in Public Health (MPH), Health Promotion, Behavioral Health, or a related field
National certifications such as:
NBC-HWC (National Board Certified Health & Wellness Coach)
CHES (Certified Health Education Specialist)
CCM (Certified Case Manager)
Experience working with underserved, high-risk, or Medicaid populations
Familiarity with value-based care models, community health systems, or integrated care teams
