Overview

The Community Health Planner promotes and supports population health by leading community health planning initiatives, including the Community Health Assessment and Community Health Improvement Plan. The Community Health Planner engages with a variety of people to assess the health of residents of Kane County and create plans for improving health. Responsibilities include developing and implementing community health assessment and planning cycles in collaboration with Department staff, staff in other Kane County Departments, community stakeholders, elected officials, and community partners. This position is full-time at 35 hours/week.
