Overview

We are seeking a compassionate, knowledgeable, and highly motivated Health Behavior Specialist to support individuals and communities in achieving healthier lifestyles. In this role, you will assess health-related behaviors, provide one-on-one or group counseling, and develop strategies to promote positive behavior change. Your work will play a key part in advancing health equity, preventing disease, and improving long-term wellness outcomes.

🎯 Key Responsibilities:

Conduct behavioral assessments to identify health risks, needs, and goals
Develop and implement individualized behavior change plans using evidence-based strategies
Provide counseling and coaching on topics such as tobacco cessation, physical activity, nutrition, stress management, and chronic disease self-management
Collaborate with interdisciplinary teams including physicians, mental health providers, dietitians, and community health workers
Lead workshops, webinars, and group sessions on health behavior change and prevention
Track and evaluate participant progress and outcomes using digital health tools and documentation systems
Assist in the design and delivery of community health education campaigns
Maintain confidentiality and ensure services comply with health and privacy regulations (e.g., HIPAA)

âś… Qualifications:

Bachelor’s degree in Public Health, Health Education, Behavioral Science, Psychology, or related field (Master’s preferred)
2–3+ years of experience in health behavior counseling, wellness coaching, or health promotion
CHES, MCHES, or Health Coach Certification preferred
Strong knowledge of behavioral change theories (e.g., Transtheoretical Model, Motivational Interviewing, Health Belief Model)
Excellent communication, motivational, and interpersonal skills
Experience working with diverse populations and addressing social determinants of health
Proficient in digital tools for remote sessions, tracking, and reporting (e.g., Zoom, EHRs, Microsoft Office/Google Workspace)
Bilingual skills are a plus
Must be authorized to work in the U.S.
